    Was one of the first two F-4Cs delivered to the USAF at MacDill AFB directly from the McDonnell factory in November 1963.

    Transferred to 58th TFTW, Luke AFB, AZ.

    Circa 1971

    Transferred to 35th TFW, George Air Force Base, CA.

    Transferred to 18th TFW, Kadena AB, Okinawa, Japan.

    From Circa 1976 to April 1984

    Transferred to 199th FIS, Hawaii ANG, Hickam AFB, HI.

    From After April 1984 to 22 July 2001

    To Kelly AFB, TX.
    Was possibly used as a BDR trainer.

    By September 2011

    To Texas Air Museum-San Antonio Chapter/Stinson Chapter, Stinson Municipal Airport/Stinson Field, San Antonio, TX.
