    Constructed as a F-104C-5-LO by Lockheed at Burbank, California, USA.

    Circa 1957

    Taken on Strength/Charge with the United States Air Force with s/n 56-0914.

    By 1962

    Transferred to 435th Tactical Fighter Squadron, 479th Tactical Fighter Wing, George AFB, CA.

    September 1962

    Flown by the winning pilot of the 1962 USAF William Tell Fighter Weapons Meet competition.

    14 October 1965

    Base of operations changed.
    Deployed to Da Nang, South Vietnam. Primary role at this time was escorting EC-121Ds over Gulf of Tonkin.

    21 November 1965

    Base of operations changed.
    Deployed to Kung Kuan AB, Taiwan.

    December 1965

    Base of operations changed.
    Returned to George AFB, CA.

    From 6 June 1966 to July 1967

    Base of operations changed.
    Deployed to Udorn RTAFB, Thailand, attached to 8th Tactical Fighter Wing. Provided escort for F-105s. Was one of first F-104s to be painted in camouflage.

    July 1967

    Base of operations changed.
    Returned to George AFB, CA.

    From By 1971 to 1975

    Transferred to 198th Tactical Fighter Squadron, Puerto Rico ANG, Muiz Air National Guard Base.

    August 1975

    Ferry flight. Delivered to National Museum of the United States Air Force, Wright Field, Dayton, OH.
