    Taken on Strength/Charge with the United States Air Force with s/n 65-0777.

    21 April 1967

    Damaged.
    In a midair collision with F-4D 65-0664 near Crestview/Eglin AFB, FL. 0777 successfully returned to base but 65-0664 crashed. Fate of crew unknown.

    Transferred to 555th TFS, 8th TFW, Ubon RTAFB, Thailand.
    Operated with markings: FY
