During a formation take-off, the engine lost power. The pilot entered an autorotation at low air speed, low RPM and low altitude resulted in a hard landing. Of the four crewmen, only 1 was injured. |

Involved in an incident.
The aircraft was being test flown for an engine and main rotor head change. The test pilot cranked the aircraft up and ran it up for the main rotor blades to be tracked. After this was completed, he picked it up to a stabilized hover with the nose into the wind. He then made a cyclic hover to the left and then a cyclic hover to the right. He then made a right pedal turn of 180 degrees and then started a left pedal turn when the aircraft started what he thought was a 1 to 1 vertical vibration. This vibration increased in severity until he lost all ground reference and abruptly lowered the collective causing the aircraft to land hard damaging the aft cross tube and undercarriage of the aircraft on the right rear side. The pilot told the maintenance personnel aboard to get out and he then shut the aircraft down. |
