    Taken on Strength/Charge with the Royal Canadian Navy with s/n 4038 and operated with these markings: 438.

    From September 1968 to Circa 1970

    Transferred to Experimental Air Squadron VX10, HMAS Shearwater, Nova-Scotia.
    Operated with markings: 438

    27 July 1970

    Taken on Strength/Charge with the Canadian Armed Forces with s/n 12438 and operated with these markings: 438.

    From 1975 to 1976

    Converted to a CH-124A by United Aircraft of Canada.
    SKIP upgrade.

    By 1983

    Transferred to 423 Maritime Helicopter Squadron, CFB Shearwater.
    Operated with markings: 438

    1983

    Base of operations changed.
    Deployed aboard HMCS Skeena.

    1991

    Received Gulf War modifications for Operation Friction and deployed for Iraq.

    From 1992 to 1993

    Operated from HMCS Preserver during Operation Deliverance (Canadian Forces support for UN peace keeping efforts in Somalia), with nose FLIR mount.

    1995

    Base of operations changed.
    Returned to CFB Shearwater.

    2006

    Base of operations changed.
    Deployed aboard HMCS Athabaska.

    2 February 2006

    Category A Crash.
    Crashed in North Sea in open water, at the Kattegat, NW of the island of Anholt, Denmark, while operating from HMCS Athabaska in NATO exercise. Ship was assigned to Standing NATO Maritime Group 1. Tail rotor struck water during flare while attempting night landing on board. Had been waved off previously, apparently due to problems on ship deck. Sank in 16 meters of water, all crew recovered by boat from HMCS Athabaska. Hoisted to surface next day, taken to harbour of Aarhus, Denmark on barge.

    Transported by ship.
    CFB Shearwater for possible repair.

    Transported by ground. Delivered to CFB Wainwright, AB.
    Used as a training aid.
