    First flight.

    6 February 1964

    Taken on Strength/Charge with the United States Air Force with s/n 63-7441.

    1965

    To 40th Flight Test Squadron, 33rd TFW, Eglin Air Force Base, FL.

    Circa 1967

    Transferred to 433rd TFS, 8th TFW, Ubon RTAFB, Thailand.
    Operated with markings: FG

    Transferred to 559th TFS, 12th TFW, Cam Ranh Bay, SVN.
    Operated with markings: XN

    27 January 1969

    Destroyed in a crash.
    Left runway and hit barrier housing during night takeoff from Cam Ranh Bay, South Vietnam and crashed into several other parked F-4s on Jan 27, 1969. Maj. Robert Came, pilot, and Capt. Thomas Burge, WSO, were killed in the crash. Came played football at the US Naval Academy prior to joining the USAF.
