    Taken on Strength/Charge with the United States Air Force with s/n 66-8759.

    By 1968

    To 480th TFS, 366th TFW, Da Nang AB, South Vietnam.
    Markings added: CV

    At some point was modified to a Pave Phantom F-4D with AN/ARN-92 LORAN-D equipment. Aerial is visible on the top of the fuselage, known colloquially as the towel-rack. Used to provide precise night navigational capability for the seeding of the Ho Chi Minh trail with sensors in the Vietnam war.

    Transferred to 4485th TS, USAF Tactical Air Warfare Center, Eglin AFB, FL.
    Operated with markings: EG

    By 1976

    Transferred to Air Development Test Center (ADTC), AFSC, Eglin AFB, FL.

    By 1977

    Transferred to 23rd TFS, 52nd TFW, Spangdahlem AB, West Germany.
    Operated with markings: SP
