    Transferred to Naval Air Rework Facility (NARF), Pensacola, FL.

    25 March 1968

    Transferred to Naval Auxiliary Air Station, Fallon, NV.

    Aircraft was used primarily for search and rescue in the area, including taking patients to and from hospitals between small towns and Reno. Aircraft was also fitted with spraying apparatus for fighting mosquitoes around NAS Fallon.

    Converted to ITASCA-Winnebago Heli-Home.Heli-Home brochure: Winnebago Industries converted this aircraft as a prototype for marketing Heli-Home/Heli-Camper, after remanufacture of the airframe by Orlando Helicopter Airways. It has sleeping facilities for four, 4 cubic foot electric refrigerator, 2-burner electric range, 40,000 BTU furnace, dual hot water heaters, shower, wash basin, toilet, 6000 BTU a/c, 3500 watt generator, dual storage batteries, 110 volt ground plug-in, full beverage service center, color TV, AM/FM stereo and tape system, two water tanks, two holding tanks, full carpeting and soundproofing, detachable awning and screen enclosure. Intended for use as RV, field office, etc., with various interiors available in production models, which would also be built from S-55 types.

    Certificate of airworthiness for N4582B (S-58H, 14-8079) issued.

    By June 1991

    To Brad Ladue/5 State Helicopters, Royse City, TX keeping c/r N4582B.

    20 June 1991

    Crashed.
    The accident occurred at Corsicana, TX, United States while cruising. The damage was substantial. The helicopter was in cruise flight over a lake when an engine power loss occurred. The pilot entered autorotation and maneuvered downwind toward the shore line. The helicopter landed hard and the right main gear strut collapsed, penetrating the fuselage. Engine examination showed a total failure of the magneto. It had recently undergone repair and inspection at an engine shop.
