    18 September 1962

    Redesignated as CH-34A.

    January 1964

    Converted to a CH-34C.

    May 1971

    Struck off Strength/Charge from the United States Army.
    SOC Atlanta Government Depot, GA.

    From July 1971 to June 1974

    Transferred to Mississippi Army National Guard.

    February 1978

    To Aircrane, Inc with new c/r N45726.

    March 1978

    To Orlando Helicopters, FL keeping c/r N45726.

    30 October 1981

    Certificate of airworthiness for NR45726 (H-34J, 56-4307) issued.

    23 December 1988

    Crashed.
    The accident occurred at the Kissimmee Airport (ISM) Kissimmee, FL, United States while landing. The damage was substantial. The pilot stated that he was repositioning the aircraft for fuel and during a practice autorotation attempt the engine hydro-mechanical clutch failed. It did not re-engage upon the application of power and the aircraft was landed hard.
