    Constructed as an UH-1H-BF by Bell at Fort Worth, Texas, USA.

    February 1969

    Taken on Strength/Charge with the United States Army with s/n 68-15369.

    March 1969

    To HHD, 44th Engineering Group, Korat RTAFB, Thailand.

    April 1969

    Transferred to 11th Armored CAV. Regiment, Bien Hoa, South Vietnam.

    9 October 1969

    Involved in an incident.
    At 2000 hours on 9 Oct. 1969, WO1 Gingrich was making a 090 deg approach to Soul Pony pad, Bien Hoa Army base. He was at an estimated 85 feet, 30 knots and 1/8 mile from touchdown when the engine failed. There was an immediate loss of RPM and the pilot lowered the collective and entered autorotation. WO1 Gingrich did not flare the aircraft but did apply collective pitch to cushion his landing. The aircrafts right skid stuck in the mud of a small ditch on touchdown causing the cross tubes to collapse. The results were major damage to the right skid, cross tubes and the underside of the fuselage. WO1 Gingrich was not injured.

    After 9 October 1969

    Transferred to 166th Transportation Corp. Detachment, 34th Group, South Vietnam.
    Prepped for return to CONUS for repairs.

    To Army Aeronautical Depot Maintenance Center (ARADMAC), NAS Corpus Christi, TX.

    April 1970

    To Bell Helicopter Company, Fort Worth, TX.

    September 1970

    Transferred to 118th Assault Helicopter Company, Bien Hoa, South Vietnam.

    January 1971

    Transferred to 173rd Assault Helicopter Company, South Vietnam.

    March 1971

    Assigned to Red River Army Depot, TX for storage.

    May 1971

    To Army Aeronautical Depot Maintenance Center (ARADMAC), NAS Corpus Christi, TX.

    August 1971

    Transferred to 8th Squadron (Air Cavalry), 1st Armored Division, 1st Army, Fort Knox, KY.

    January 1972

    Transferred to U. S. Army Aviation School, CONARC, Fort Rucker, AL.

    July 1973

    Transferred to U. S. Army Aviation School, 1st Army, Fort Rucker, AL.

    September 1975

    Loaned to NASA Langley Research Center, Hampton, VA.
