    UH-1D 64-13639 was on a medical evacuation rescue and recovery mission in South Vietnam when it came under fire while descending at 30 feet and 0 knots. The helicopter sustained four hits from 7.62mm small arms or automatic weapons fire, specifically in the passenger cargo section. The attack caused personnel casualties, although the aircraft itself remained operational and continued flying. However, due to the incident, the mission was diverted and could not achieve its intended objectives.

    UH-1D 64-13639 was involved in a combat incident during a medical evacuation rescue and recovery mission in South Vietnam. While landing at 10 feet and 10 knots, the helicopter was struck five times by 7.62mm small arms or automatic weapons fire, with hits concentrated in the equipment section. The attack damaged critical systems including the electrical, engine, and hydraulic systems. Although the helicopter initially continued flight, it ultimately made a precautionary landing and was later recovered by means other than its own power. Both the mission and flight capability were terminated, though the aircraft was later repaired in-theater.
