    24 May 1970

    Damaged.
    AH-1G 67-15829 was involved in a combat incident in South Vietnam while conducting close air support. During an attack run at 800 feet and 140 knots, the helicopter sustained three hits from 7.62mm small arms fire, impacting the tail section and damaging structural components. Despite the damage, the aircraft continued flight, completed its mission objectives, and was later repaired in theater.

    Involved in an incident.
    While starting up, AH-1G 67-15829 suffered a weapons malfunction that fired a salvo of rockets due to defective armament system. 20 year old SP4 Leal Salvador, Jr. was killed in the accident.

    20 November 1970

    Damaged.
    AH-1G 67-15829 was involved in a combat incident in South Vietnam during a close air support mission. While en route in level flight at 500 feet and 100 knots, the helicopter was hit by 7.62mm small arms fire. Rounds struck the canopy, causing spalling and resulting in structural damage and two crew members wounded in action (WIA). Despite the damage and injuries, the helicopter continued flight and was later repaired in theater.

    22 January 1971

    Damaged.
    AH-1G 67-15829 sustained combat damage during an armed reconnaissance mission in South Vietnam. While en route at 700 feet and 90 knots, the helicopter took two hits from 7.62mm small arms fire, impacting the main rotor blade system and damaging the armament and rotor systems. Despite the damage, the helicopter continued flight, completed its mission objectives, and was repaired in theater.

    9 April 1971

    Damaged.
    AH-1G 67-15829 experienced a flight control issue during a rocket run in South Vietnam. The aircraft initiated the attack from 1,000 feet AGL at 80 knots with a 25-degree descent angle. At 500 feet and 120 knots, the pilot applied 42 lbs of torque and noticed rotor RPM bleeding off as the aircraft began to settle. To avoid terrain ahead, torque was increased to 50-51 lbs, but RPM dropped further to 6000. The helicopter flew through the tops of trees on a ridgeline, resulting in damage to both the main rotor and tail rotor blades.
