    Crashed.
    The accident occurred at Gonzales, CA, United States while maneuvering. The damage was substantial. A loss of power was expereinced as the aircraft descended into a field at the beginning of an aerial application spray run. The pilot initiated a forced landing into the field. During the landing the right skid dug into the field, tipping the helicopter onto its right side. An examination of the wreckage revealed that there was no evidence of fuel in the aircraft fuel tank...

    Crashed.
    The accident occurred at Watsonville, CA, United States while taking off. The damage was substantial. The certificated commercial pilot was conducting aerial spray operations in a helicopter. About 30 minutes after departing a nearby airport, the pilot was taking off from a field to begin applying the third load of an insecticide. About 100 yards from the takeoff point, the helicopters engine suddenly lost power and sounded as if it was misfiring...

    Crashed.
    The accident occurred at Salinas, CA, United States while maneuvering. The airframe was destroyed. The pilot lost control of the helicopter while executing a maneuvering turn while loaded with chemical at about 35 feet above the ground. The main rotor head paddle assembly fractured and separated before the loss of control. Metallurgic examination of the fracture revealed an overload failure mode.
