    Constructed as an UH-1D-BF by Bell at Fort Worth, Texas, USA.

    April 1966

    Taken on Strength/Charge with the United States Air Force with s/n 65-9803.

    By September 1966

    To 175th Aviation Company, South Vietnam.

    15 September 1966

    Damaged.
    Torque indicator registered zero followed by odor of burning oil. Loud pop from engine and complete power fail. The pilot managed to land aircraft safely. Airframe and crew of UH-1D 65-9803 were evacuated by CH-47. Cause of crash was the failure of No. 2 turbine shaft bearing.

    11 October 1966

    Damaged.
    While in-flight, UH-1D 65-9803 experienced engine failure. The airframe suffered major damage upon landing. Crew survived but was injured.

    November 1966

    Transferred to 611th Transportation Company, South Vietnam.

    March 1967

    Transferred to 388th Transportation Company, South Vietnam.

    June 1967

    Transferred to 330th Transportation Company, South Vietnam.

    December 1969

    Transferred to 388th Transportation Company, South Vietnam.

    May 1970

    Transferred to 336th Assault Helicopter Company, South Vietnam.

    July 1970

    Transferred to 388th Transportation Company, South Vietnam.

    August 1970

    To U. S. Army Transportation Aeronautical Depot Maintenance Center (ARADMAC), NAS Corpus Christi, TX.

    January 1971

    Transferred to U. S. Army Aviation Division, 3rd Army, Fort Gordon, GA.

    September 1971

    Transferred to 11th Special Forces Group (Aviation Battalion), Miller AAF, NY.

    January 1972

    Transferred to U. S. Army Aviation School, Fort Stewart, GA.

    March 1972

    Transferred to Supply Division, 3rd Army, Hunter AAF, GA.

    April 1972

    Transferred to 197th Aviation Company, 3rd Army, Fort Benning, GA.

    June 1972

    Transferred to U. S. Army National Guard.

    Converted to an UH-1H-BF by Bell at Fort Worth, Texas, USA.
