    Crashed.
    The incident occurred at Panama City, FL, United States. The time was 1005 hours. The damage was substantial. Incident occurred at the Edgewater Beach Airport. The tail rotor failed while taking off in the vertical. Suffered a hard landing on touch down. The emergency circumstances included; forced landing on a seaplane base or heliport and prop engine vibration. Report remarks include: Local sightseeing flts. Failed t/right blade not recovered.

    Crashed.
    The incident occurred at Panama City, FL, United States. The time was 1640 hours. The damage was substantial. This was a local flight from Panama City, FL. The tail rotor failed during the initial climb. Ditched while performing a power-off autorotative landing. The factors included; separation in flight and came to rest in water. The emergency circumstances included; forced off-airport landing on water. Report remarks include: T/right blade retaining bolt, pn47-461-052-3, failed...

    3 July 1983

    Crashed.
    The accident occurred at Thomasville, IL, United States while landing. The damage was substantial. The helicopter tail rotor struck the ground during landing. The helicopter was engaged in aerial application and was returning to reload. The pilot reported that at touchdown a gust of wind caused the tail rotor to hit the ground. Decatur winds were reported as from 190 degrees at 12 knots at 0750 cdt. No gusts reported.
