    Summarized NTSB narrative from report number SEA76DYE14: There were no fatalities. Incident occurred at 07:48 hours. The airframe was destroyed. This was a local flight from Randle, WA. Suffered a hard landing while landing. The gear collapsed while landing. The factors included; overload failure and rough uneven terrain. Incident occurred at or near Randle, WA.

    Crashed.
    There were three fatalities. The accident occurred at Applegate, OR, United States while taking off. The airframe was destroyed. The helicopter landed on a mountainside helipad where two passengers were waiting. The pilot of a sikorsky s 61 helicopter flying nearby cautioned him about clouds moving up the valley. He also noted it was quite windy and turbulent. The pilot replied he was aware of the approaching clouds and that it was bad...
