    Constructed as a F-105D-25-RE by Republic at Farmindale, New York, USA.

    13 November 1962

    Taken on Strength/Charge with the United States Air Force with s/n 61-0188.

    20 December 1962

    To 18th Tactical Fighter Wing, Kadena AB, Okinawa.

    20 December 1964

    Due to pilot error, the aircraft blew a tire on landing and engaged the barrier at Kadena AB. The pilot was Maj. James W. Mathews.

    By August 1965

    Transferred to 67th TFS, 18th TFW, Kadena AB, Okinawa.

    17 August 1965

    Base of operations changed.
    Deployed to Korat RTAFB, Thailand.

    12 September 1965

    Damaged.
    Flown as Elm 3 by Capt. Jack L. Francisco on a combat mission against the Yen Khoai barracks in North Vietnam. The aircraft was hit in the radome by flak. Flight lead was the 67th TFS c/o Lt. Col. Robinson Risner.

    16 September 1965

    Flown as Oak 2 by Capt. Goode on an Iron hand mission against a SAM site along Route Package 4 in North Vietnam. On this mission, Maj. Raymond James Merritt was flying as Oak 1 (F-105D 61-0189) and shot down by flak. Lt. Col. Robinson Risner, flying as Pepper 1 (F-105D 61-0127), was also shot down. Both of these 67th TFS pilots became POWs. Capt. Goode flew RESCAP for both pilots, then flew to Da Nang AB, South Vietnam, when he ran out of fuel.

    17 September 1965

    Capt. Goode flew from Da Nang to Korat with the callsign Soup 1.

    15 July 1967

    Transferred to 355th TFW, Takhli RTAFB, Thailand.

    2 September 1967

    Damaged.
    While flying on a combat mission, the aircraft received battle damage from enemy flak. The inboard pylons were damaged beyond repair and six feet of the leading edge flap was blown off. Aircraft returned to Takhli and was repaired.

    29 June 1968

    Damaged.
    On a combat mission over North Vietnam, the aircraft recieved battle damage when two M-117 bombs struck the lower left and right side of the fuselage causing substantial damage. Aircraft returned to Takhli and was repaired.

    By January 1970

    Transferred to 354th TFS, 355th TFW, Takhli RTAFB, Thailand.
    Operated with markings: Stephanie Allison
    Assigned to 1st Lt. Bruce Cox, he named the aircraft Stephanie Allison after his daughter. Cox would fly this jet on more than 15 combat missions in early-mid 1970.

    12 January 1971

    Transferred to 18th TFW, Kadena AB, Okinawa.

    19 March 1971

    Transferred to 23rd TFW, McConnell AFB, KS.

    From 31 March 1971 to 24 May 1971

    Assigned to McConnell AFB, KS for storage.

    26 July 1971

    Transferred to 121st TFS, 113th TFW, Washington D. C. ANG, Andrews AFB, MD.

    By September 1977

    To National Museum of the United States Air Force Loan Program, Wright Field, Dayton, OH.
