    Certificate of airworthiness for N49086 (PT-22, 1541) issued.

    21 June 1982

    Crashed.
    There was one fatality and was one serious injury. The accident occurred at Haigh Field Orland, CA, United States while taking off. The airframe was destroyed. At about 50 feet in the air the aircraft turned left straight down.

    2 February 1984

    Crashed.
    The accident occurred at the El Paso International Airport (ELP) El Paso, TX, United States while taking off. The damage was substantial. Investigation revealed a touch-and-go landing was completed when the aircraft became airbourne prematurely and drifted slowly left in a light crosswind. After flying about 2000 feet the main gear struck the top of a sand dune and the aircraft nosed over.
