    Constructed as an UH-1.

    April 1968

    Taken on Strength/Charge with the United States Army with s/n 67-17355.
    The delivery data is provided. It was delivered to the Army at Forth Worth, TX.

    May 1968

    Transferred to 48th Assault Helicopter Comany (Blue Stars), Vietnam.
    Operated as an armed troop carrier for 13 months.

    June 1969

    Crashed.
    Shot down after sustaining multiple hostile fire rounds in the transmission and engine area.

    Recovered.

    Transferred to Army Aeronautical Depot Maintenance Center (ARADMAC), Corpus Christi Army Depot (CCAD).
    Sent to the CCAD for a rebuild.

    1970

    Transferred to 351st Aviation Company, Germany.

    1973

    Transferred to 71st Aviation Company, Germany.

    1974

    Transferred to 582nd Transportation Company, Germany.

    1974

    Transferred to 24th Engineering Group, HHC, Germany.

    1987

    Transferred to 56th Aviation Company, 70th Transportation Battalion, Coleman Barracks, Mannheim, Germany.

    From Unknown to 1995

    Transferred to 86th Army Reserve Command, Naval Air Station Glenview.
    Assignement to the 86th ARC is not confirmed.

    To Volo Antique Auto Museum, Volo, IL.
