    16 November 1982

    Involved in an incident.
    The incident occurred at Huntsville, AL, United States while on decent. The damage was minor. The left cargo door separated from the aircraft. The door was a hamilton cargo door installed under an stc which required 3 hinges on top 3 pins on bottom with an overcenter locking mechanism on the handle for the pins. This door had 2 hinges installed. Only 2 pins were found operational there was no operable overcenter locking mechanism.

    6 June 1983

    Crashed.
    The accident occurred at the Jacksonville Airport (JAX) Jacksonville, FL, United States while landing. The damage was substantial. The pilot reported that during the landing roll, while rolling on all 3 gear, the tailwheel failed steered sideways causing the aircraft to veer off the runway. An investigation revealed the tailwheel truss axle had failed. A further inspection revealed that 50 of the fracture surface was rusted...
