    Constructed as an UH-1D-BF by Bell at Fort Worth, Texas, USA.

    November 1965

    Taken on Strength/Charge with the United States Army with s/n 64-13882.

    29 June 1966

    Damaged.
    During a night flight, the air intake cover was not removed during the pre-flight inspection. The CC noticed the cover after the engine had started. Damage required the short shaft to be changed.

    13 September 1966

    Damaged.
    While taking off from a hot LZ in South Vietnam 500 feet and UNK knots, UH-1D 64-13882 was hit once in the engine compartment by enemy ground fire. The helicopter continued flight and accomplished all mission objectives.

    19 October 1966

    Damaged.
    Explosion heard in engine compartment. Application of coll resulted in loss of RPM. Completed auto sus short shaft fail preformed packing failed to keep greas in aft end of short shaft after 95 hrs flt lack of lube and cooling caused short shaft fail.

    October 1966

    To Bell Helicopter Company, Fort Worth, TX.

    November 1966

    Transferred to Troop C, 1-9 CAV., 1st Cavalry Division, South Vietnam.

    15 November 1966

    Damaged.
    While taking off from a hot LZ in South Vietnam, UH-1D 64-13882 was at 5 feet and UNK knots when it took a single 7.62MM round to the fuel tank. The self sealing fuel tank developed a leak. The helicopter continued flight, however, the aircraft was diverted prior to accomplishing any mission objectives.

    Transferred to Post Supply, 6th Army, Fort Ord, CA.

    Transferred to Army Aviation Division, 6th Army, Dugway Proving Ground, UT.

    Transferred to U. S. Army-Vietnam Flight Det. (USARV), South Vietnam.

    14 July 1967

    Damaged.
    The main rotor blades struck a tree limb on approach to a confined area.

    August 1967

    To Bell Helicopter Company, Fort Worth, TX.

    November 1967

    Transferred to 355th Aviation Company, 4th Army, Fort Sill, OK.
    Deployed to South Vietnam.

    11 November 1967

    Damaged.
    While on a mission for spraying / defoliage over South Vietnam, this helicopter was traveling at 50 feet and UNK knots when it took a single 7.62MM hit to the equipment section.

    17 December 1967

    Damaged.
    While on a mission over South Vietnam, this helicopter was traveling at 500 feet and UNK knots when it took a single hit to the crew compartment. Apparently there was a casualty.The helicopter continued flight and accomplished all mission objectives.

    Converted to an UH-1H by Aerospace Industrial Development Corp..

    January 1971

    Transferred to U. S. Army Aviation School, Fort Stewart, GA.

    November 1971

    Transferred to Supply Division, 3rd Army, Hunter AAF, GA.

    December 1971
