    Constructed as a F-4D-25-MC by at Saint Louis, Missouri, USA.

    From January 1966 to August 1967

    Richard Phillips comments (29 December 2020): During this period I was stationed at Nellis AFB. We received F4D 64-0959 new from the factory to Fighter Weapons School. I was crew chief on this airframe. The plane was new and I was the first Air Force crew chief assigned to this aircraft. My goal was to get 100 hours without having an abort. I missed my goal by 2 hours. The 64-0959 Phantom had the best record in our fleet. When the base commander(General Pete Everest) flew to Washington D.C., he flew 64-0959.

    Operated with markings: BT

    1973

    Transferred to 23rd TFS, 52nd TFW, Spangdahlem AB, Germany.
    Operated with markings: SP

    26 December 1986

    On 26 December 1986, this jet was standing alert at the Fargo ANGB when it was released from alert status to fly a heart from a infant donor in Fargo to a toddler recipient in California. 0959 flew from Fargo to Hill AFB and then to San Diego to deliver the heart for transplant. The heart recipient survived and is now a motivational speaker.
