    Constructed as an UH-12E.

    To unknown owner with c/r N4029D.

    4 April 1988

    Crashed.
    The accident occurred at La Salle, TX, United States while maneuvering. The damage was substantial. Aircraft was substantially damaged during an emergency autorotation following an engine failure while the aircraft was completing an aerial application maneuver. Aircraft touched down tail low and the main rotor blades flexed down and cut off the tail boom. Investigation revealed water in the carb bowl, the aircraft fuel tank and the refueling vehicle.

    Certificate of airworthiness for N4029D issued.

    11 October 1991

    Crashed.
    The accident occurred at Palacios, TX, United States while taking off. The damage was substantial. The commercial helicopter pilot was departing a trailer on an agricultural application flight. A skid remained tied to the trailer when a takeoff was attempted. Control of the helicopter was lost and it collided with the terrain adjacent to the trailer.
