    Constructed as an AH-1G-BF by Bell at Fort Worth, Texas, USA.

    June 1969

    Taken on Strength/Charge with the United States Army with s/n 68-15169.

    August 1969

    To 11th Armored Cavalry Regiment, South Vietnam.

    11 September 1969

    Shot down.
    During a recon for Armed Recon mission over South Vietnam,AH-1G 68-15169 was traveling at 100 feet and 120 knots when it took 1 hit from an enemy 7.62MM weapon. The fuel systems and structure were critically damaged, making a forced landing unavoidable. Both mission and flight capability were terminated. The helicopter was later recovered by means other than its own power.

    18 May 1970

    Shot down.
    During a close air support (CAS) mission over South Vietnam, AH-1G 68-15169 was attacking at 1000 feet and 110 knots. Helicopter took 3 hits from an enemy 7.62MM weapon. The helicopter made a Forced Landing. Aircraft is later recovered by any means other than its own power. Both mission and flight capability were terminated.

    January 1971

    Transferred to HHT, 11th Armored Cavalry Regiment, South Vietnam.

    March 1972

    Returned to United States.

    July 1972

    To United States Army Aeronautical Depot Maintenance Center (ARADMAC), NAS Corpus Christi, TX.

    January 1973

    Loaned to U. S. Army Aviation Systems Command (AVSCOM), Saint. Louis, MO.

    February 1973

    Returned from loan to ARADMAC, NAS Corpus Christi, TX.

    July 1973

    Transferred to Supply Division, 5th Army, Fort Knox, KY.

    September 1973

    Transferred to 235th Assault Helicopter Company, 1st Army, Fort Knox, KY.

    Converted to an AH-1S.

    December 1973

    Transferred to Supply Division, 5th Army, Fort Knox, KY.

    March 1974

    Transferred to C Troop, 7-1 CAV., 5th Army, Fort Knox, KY.

    April 1974

    Transferred to Supply Division, 5th Army, Fort Knox, KY.

    October 1974

    Transferred to C Troop, 7-1 CAV., 5th Army, Fort Knox, KY.

    By August 1994

    Transferred to 1-147th Aviation Battalion, Wisconsin Army National Guard, Truax Field, Madison, WI.
