    Constructed as a C-135B-BN by Boeing at Renton, WA, USA.

    17 August 1962

    First flight.

    Taken on Strength/Charge with the United States Air Force with s/n 62-4139.

    29 August 1962

    Transferred to 1611th Air Transport Wing (MATS), McGuire AFB, NJ.

    14 December 1967

    Converted to a RC-135M.
    Date conversion began, last C-135B converted to RC-135M.

    1968

    Transferred to 82nd Reconnaissance Squadron, Kadena AB, Okinawa.

    Flew 1000th Combat Apple Sortie during Vietnam War.

    May 1976

    Aircraft re-skinned.

    17 August 1979

    Damaged.
    Involved in take-off accident at Clark AB, Philippines. Although some reports stated that the aircraft was written off, it was repaired and resumed operations, but with a slightly twisted fuselage, resulting in the plane flying with perpetual yaw.

    Ron McMasters comments (4 February 2023): Aircraft abort at takeoff due to hydroplaning. Only thrust reversers1, 2, and 3, were engaged causing asymmetric thrust, aicraft rotated 180 degrees as it departed runway, causing right landing gear to collapse and impale right wing at edge of fuselage. Landing gear, gear doors, and engines 3 and 4 were damaged. Aircraft remained at Clark AB for nearly two years for repair until it could be flown back to Offutt AFB for further repairs. After return to service, it became known as the Sidewinder due to its perpetual yaw.
