    By October 1966

    To Troop B, 1-9 CAV., 1st Cavalry Division, South Vietnam.

    1 June 1967

    Damaged.
    During a Recon mission for Armed Recon over South Vietnam, OH-13S 64-15393 was at Level Flight at 100 feet and 60 knots when it took 4 hits from an enemy 7.62MM weapon. The helicopter was hit in the bottom, damaging the structure. The helicopter continued flight and accomplished all mission objectives.

    6 October 1967

    Damaged.
    During a Recon mission for Armed Recon over South Vietnam, OH-13S 64-15393 was at Level Flight at 900 feet and 60 knots when it took 1 hit from an enemy 7.62MM weapon. The helicopter was hit on the right side, damaging the structure and landing skid. The helicopter continued flight and accomplished all mission objectives.

    21 October 1967

    Damaged.
    During a Recon mission for Armed Recon over South Vietnam, OH-13S 64-15393 was at Level Flight at 5 feet and 50 knots when it took 2 hits from an enemy 7.62MM weapon. The communication systems and flight controls were damaged. The helicopter continued flight and accomplished all mission objectives.

    Transferred to 355th Aviation Company, South Vietnam.

    December 1967

    Transferred to HHC, 3rd Brigade, 1st CAV. Division, South Vietnam.

    January 1968

    Transferred to 355th Aviation Company, South Vietnam.

    By April 1968

    To Bell Helicopter Company, Fort Worth, TX.

    May 1968

    Transferred to 355th Aviation Company, 4th Army, Fort Sill, OK.

    July 1968

    Transferred to OPM SEA NITEOPS, 1st Army, Fort Belvoir, VA.

    August 1968

    Transferred to HHS, 44th Engineering Group, Korat RTAFB, Thailand.

    After November 1968

    Returned to the United States.

    By March 1969

    Assigned to Sharpe Army Depot, CA for storage.

    By January 1970

    Transferred to VII Corps, SPT COM, West Germany.

    May 1970

    Transferred to Red River Army Depot, 4th Army, TX.
