    Constructed as an UH-1D-BF by Bell at Fort Worth, Texas, USA.

    Circa November 1965

    Taken on Strength/Charge with the United States Army with s/n 64-13879.

    October 1966

    To 79th Transportation Company, South Vietnam.

    Circa 1966

    Damaged.
    This was a combat incident. During a troop extraction mission in South Vietnam, the landing zone was hot. Taking off at 50 feet and UNK knots, this helicopter took a single hit from an enemy 7.62MM caliber weapon, likely an AK-47. The communication systems and airframe structure were damaged. The aircraft continued flight and accomplished all mission objectives.

    Transferred to 162nd Assault Helicopter Company, South Vietnam.

    14 July 1967

    Damaged.
    This was a combat incident. During an unarmed combat recon mission, this helo took a single hit by an enemy 7.62MM weapon. The bullet struck the helicopters fuel system, puncturing the self-sealing fuel tank. The fuel tank sealed automatically. The helicopter continued flight and accomplished all mission objectives.

    Assigned to ARADMAC, 4th Army, NAS Corpus Christi, TX for storage.

    March 1969

    Transferred to HHD, 44th Engineering Group, Korat, Thailand.

    September 1969

    To 207th Aviation Company, 15th AG, Heidelberg AAF, West Germany.

    January 1971

    Transferred to 207th Aviation Company, Heidelberg AAF, West Germany.

    February 1971

    Transferred to HHC, 7th Engineering Brigade, West Germany.

    March 1972

    Transferred to 349th Aviation Company, West Germany.

    July 1972

    Transferred to ARADMAC, 4th Army, NAS Corpus Christi, TX.

    August 1972

    Transferred to Red River Army Depot, 5th Army, Bowie County, TX.

    February 1973

    Loaned to U. S. Navy, NAS Patuxent, MD.

    May 1973

    Loaned to NASA, Langley Research Center, Hampton, VA.

    July 1973

    Transferred to U. S. Army National Guard.
