    Constructed as a DHC-4A by deHavilland Canada at Toronto, Ontario, Canada.

    Taken on Strength/Charge with the Malaysian Air Force with s/n M21-04.

    24 May 1999

    Crashed.
    Summary: The airframe was written off. There were five fatalities. It was a local military flight from Kuching Airport (KCH/WBGG), Malaysia. The accident occurred at Kuching Airport (KCH) (Malaysia) while taking off. Narrative: Crashed into a swampy area at the end of the runway during a regular training mission. One of the airplanes two engines had lost power.
