    Crashed.
    The accident occurred at Dummerston, VT, United States while maneuvering. The damage was substantial. The pilot initiated a practice autorotation from about 600 feet above ground level. Airspeed was about 55 knots with low m/right RPM due to low gross weight. A moderate flare was initiated in an effort to increase rotor RPM, reducing the airspeed to 40 knots. However, the application of collective failed to arrest the descent rate the t/right struck the ground causing the helicopter to vibrate...

    Crashed.
    There was one fatality. The accident occurred at Hammonton, NJ, United States while cruising. The airframe was destroyed. At the end of an aerial application mission, the helicopter was refueled and the pilot departed for home base. A few minutes after takeoff, the helicopter collided with trees. Examination of the engine and drive train revealed no preimpact failure; however, the collective pitch control yoke bearing had failed...

    There was one fatality. The accident occurred at Portia, AR, United States while taking off. The damage was substantial. On May 13, 2016, about 0715 central daylight time, a Bell 47G helicopter, N48316, collided with a service truck during takeoff near Portia, Arkansas. One person on the ground was fatally injured. The helicopter was substantially damaged and the commercial rated pilot was not injured...

    Crashed.
    There was one fatality. The accident occurred at Portia, AR, United States while taking off. The damage was substantial. While departing from an elevated wood platform, the pilot elected to make a sliding takeoff and after the helicopters skids had slid forward about 2. 5 feet, the skid plate on the bottom of the helicopters right skid caught on a raised deck screw that had backed out of the platform. The pilot attempted to free the skid by applying power and pitching the helicopter forward...
