    Constructed as an UH-1D-BF by Bell at Fort Worth, Texas, USA.

    November 1965

    Taken on Strength/Charge with the United States Army with s/n 64-13866.

    By September 1966

    Deployed to South Vietnam.

    6 September 1966

    Damaged.
    During an air/land assault into a hot LZ in South Vietnam, UH-1D 64-13866 was in level flight at 30 feet and 80 knots when it took 2 hits from an enemy 7.62MM weapon. The rounds damaged the structure and wounded a crewmember. The helicopter continued flight and accomplished all mission objectives.

    By October 1966

    Transferred to 162nd Assault Helicopter Company, Phouc Vinh, South Vietnam.

    5 January 1967

    Damaged.
    During a troop extraction mission from a hot LZ in South Vietnam, UH-1D 64-13866 was landing at 200 feet and 75 knots when it took 6 hits from a concealed mine and an enemy 7.62MM weapon. The helicopter was hit in the cockpit. The armor protecting the personnel was effective. The helicopter continued flight, however, was diverted prior to accomplishing any mission objectives.

    18 March 1967

    Damaged.
    During a troop extraction mission from a hot LZ in South Vietnam, UH-1D 64-13866 was landing at 75 feet and 50 knots when it took 1 hit from an enemy 7.62MM round in the cockpit. The helicopter continued flight and accomplished all mission objectives.

    May 1968

    Returned to the United States.

    June 1968

    Transferred to 355th Aviation Company, 4th Army, Fort Sill, OK.

    July 1968

    Assigned to Army Aeronautical Depot Maintenance Center (ARADMAC), NAS Corpus Christi, TX for storage.

    November 1968

    Transferred to Davison Army Airfield, VA.

    November 1970

    Transferred to U. S. Army National Guard.

    February 1973

    To ARADMAC, NAS Corpus Christi, TX.

    September 1973

    Loaned to NASA, Langley Air Research Center, VA.

    January 1974

    Transferred to U. S. Army National Guard.

    Converted to an UH-1H by Aerospace Industrial Development Corp..

    By September 2012

    To Chase County Veterans Memorial, Swope Park, Cottonwood Falls, KS.
